Cohorts User Guide

Akute’s Cohorts feature lets you create custom patient panels using criteria like age, ICD-10 diagnoses, health maintenance items, appointment data, and more. Once a cohort is built, you can pair it with message templates and create automations to send targeted outreach via Akute secure portal message, SMS, or email.


Step 1: Build a Cohort

To create a custom patient cohort:

1. Navigate to Cohorts under Engage in the navigation bar.

2. Click + Create Cohort.

3. Name your cohort (e.g., “Breast Cancer Screening”).

4. Add a description (e.g., “Women 50+ with no mammogram in 2 years”).

5. Add your filters:

• Use structured fields like Age, Gender, or ICD-10 codes.

• Use free-text fields like Problem List or Health Maintenance to look for missing or specific keywords (e.g., “no colonoscopy in 10 years”).


Tip: How to use logic filters:

When building cohorts, you can use filters and logic to narrow down exactly which patients you want to include. Akute supports options like:

is exactly / is not – for exact matches against a single value or entry (e.g., 

Gender is Female)

is one of / is not any of – for finding keywords across multiple values or entries (e.g., Problem List contains diabetes, DMII, DM)

contains / does not contain – for finding keywords across multiple 

values or entries (e.g., Problem List contains diabetes, DMII, DM)

<, <=, >, >=, = – for numeric or date-based filters (e.g., Age >= 65, 

Last Appointment Date (Days) < 365)

has the word(s) – for finding one or more specific words within free-text fields (e.g., Problem List has the word(s) “chronic pain”)

has none – to find patients missing a field entirely (e.g., no value 

present for Health Maintenance), equivalent to = 0 logic

AND / OR logic – combine filters to create more advanced cohorts (e.g., Age > 50 AND Problem List contains hypertension; or Problem List contains prediabetes OR Lab Value contains A1c >5.7 and <6.4)


6. Click Save to generate the cohort.

7. Click Preview to review your matching patient list

• Make sure the output aligns with your intent.

• Double-check for any false positives or missing patients.
