Emails for manually created appointments

For manually booked appointments, Akute sends an email if "Email Appointment To Patient" is selected when saving the appointment.

This email is the standard email if no appointment type was selected or if no custom email is included in the appointment type. This email is the custom email if an appointment type was selected that includes a custom email.

This email will also include the calendar invite for the patient.
