Recording Vaccines in Akute

Akute allows you to document both administered vaccines and historical vaccines directly within the patient chart. Vaccine entry lives under the Health Maintenance section and is designed to support accurate clinical documentation and downstream reporting. Akute does not currently submit vaccine data to state immunization registries. Clinics remain responsible for any required external reporting.


Where to Find Vaccine Entry

  1. Open the patient’s chart.
  2. Navigate to Health Maintenance.
  3. Click Vaccines.
  4. Select Add Vaccine.

You will be prompted to choose between Administer Vaccine or Historical Entry.

Administer Vaccine

Use Administer Vaccine when documenting a vaccine given in your clinic.

Step 1: Select Vaccine Type

  • Choose the vaccine type (e.g., Influenza, Tdap, COVID-19).
  • This selection controls which products are available for selection and ensures accurate coding.

Step 2: Select Product

  • Choose the specific vaccine product from the dropdown.

    Selecting a product automatically populates:

    • CVX/vaccine code
    • MVX/manufacturer code
    • Manufacturer name
    • Suggested dosage, when available
    • Most recent CDC Vaccine Information Statement (VIS) publication date
    • A link to the appropriate VIS

Step 3: Complete Administration Details

The following fields are required unless noted otherwise:

  • Lot Number
  • Expiration Date
  • Route
  • Site
  • Administered By
  • Ordered By
    • This defaults to the patient’s Primary Care Provider to reduce clicks
    • You may change this if needed

Optional fields:

  • Volume Administered
  • Notes

Once all required fields are complete, click Save to record the vaccine.

Historical Entry

Use Historical Entry when recording vaccines the patient received elsewhere in the past.

  • No fields are required.

    Enter as much or as little information as is known.

    This is useful when:

    • The patient reports a prior vaccine but details are incomplete
    • Only an approximate date is known
    • The vaccine type is known but not the product

Historical entries are clearly labeled and do not require product or lot selection.

Viewing Vaccine Details

From the Vaccines list:

  1. Locate the vaccine entry.
  2. Click the three-dot menu.
  3. Select View Details.

This view displays all recorded information, including product details, administration data, VIS dates, and notes.

Vaccine data will be available for use in PatientPulse cohorts in a future update.
