Create custom forms

You can create your own custom intake forms for patients to electronically fill out within Akute! These forms can be used for scoring questionnaires such as PHQs and GADs, new patient information details such as past medical history, surveys to send to patients, or anything else you'd like.

We have 2 types of forms:

  1. Custom Forms from Templates
    1. These are forms that we've already built for you. This allows you to quickly get started with your patient intake instead of having to start from the beginning.
    2. These templated forms are not yet editable, but we are working hard to make them editable for you.
  2. Custom Forms from Scratch
    1. These are customizable forms that you build from the ground up. Add different questions, answer types, required questions, etc. 
    2. Currently we offer four different answer types:
      1. Short Text
      2. Paragraph
      3. Date
      4. Multiple Choice (single answer allowed)
      5. Checkbox (multiple choice with multiple answers allowed)
      6. Score (similar to PHQ style questionnaires)
      7. Allergies
        1. Allergies are what we call "coded" answer types. Meaning, if patients answer these questions in your forms, you'll be able to add those answers natively to the patient chart with a single click of a button. This will be available when viewing the form answers in the "Forms" section inside the patient chart.
        2. Tip! Don't forget about our 3 column view option next to the patient's name at the top right of the left-side summary bar. This opens a 3rd column on the right side of the screen where you can update the chart while viewing the form answers. Also, if you're on a smaller screen, look for the "Collapse Navbar" option at the bottom left of the navigation bar. This will give you a bit more screen real-estate to view all the information.
    3. When creating a form from scratch, you have several editing options available.
      1. Setting a required question
      2. Allowing multiple answers (i.e. Can the patient add multiple answers to the same question. This is enabled by default for Allergies answer types, thus allowing patients to enter multiple allergies.
      3. Duplicate a question (next to trash can icon). This creates a duplicate question and inserts it to your form after the question that was duplicated.
      4. Delete Question. This will delete that specific question.
      5. Change order. Drag question cards to different spots in the form to change the ordering of questions.
      6. Change answer types dropdown. This allows you to change the answer type of a question.
      7. Add questions. Add a new question to the form.

Publishing Forms:

  1. You can create, edit, and delete forms from the "Forms" subsection in the settings/forms page in Akute. Just click anywhere on the card of the form you want to edit/delete. This will expand the form card and you'll see those options at the bottom right of the expanded card.


  2. Once you have created your forms and are ready for patients to access them, you can Publish your forms as a "Bundle". 
    1. A Bundle is a set of 1 or more forms that can be sent to patients as 1 single link.
    2. This prevents drop-off between forms as we seamlessly transition patients from 1 form to another in a bundle. 
  3. Once you've published your Form Bundle, you can then copy the link we create for your Bundle and send it to patients directly, add them to your website, or include the link in the appointment confirmation emails in the Appointment Types editor.
    1. The Form Bundle link can be seen at the bottom right of the bundle card when expanded. In addition, there is a copy button next to the link which will automatically copy the link for you to paste anywhere.
    2. There are also edit and delete options for each form bundle. This allows you to add/remove specific forms from a Bundle and/or delete an entire bundle. Once a Form or Bundle have been removed, patients will no longer have access to complete those forms. All existing answers will still persist and be available in the charts.
